Pre-Arrangements Form

Our intention at this site is to allow you to browse funeral options and be able to make pre arrangements for yourself or someone in your care right at your own computer. Of course Chris Ryan, Sr. is always willing to meet with you in person, but if you'd like please try this exercise.

Note: You don't have to pre pay to have your wishes known.

If you are not sure as to exactly what type of service you prefer, go to "Funeral Options" on the navigation to your left to see some choices.

This form begins with data needed to complete a death certificate and secure proper permits for burial or cremation. Next is an area to list survivor's names and details of your life that you would want in an obiturary. Page 4 begins the area where you can check off your specific instructions. Be as detailed as you want. Add extra notes. Notice that not all items here will apply to every arrangement.

After completing this Pre Arrangement process click SEND to have your instructions on file at Gleeson-Ryan Funeral Home. You will join the hundreds of other pre arrangements that are safely and confidentially held at Gleeson-Ryan Funeral Home. You also have the opportunity to print a copy to give to a responsible family member or caregiver.
